Addiction Aftercare & Recovery Support

Treatment is the start. Aftercare is where it holds.

Leaving residential or outpatient treatment is a vulnerable moment. Aftercare gives you ongoing one-to-one support and accountability so progress made in treatment carries into everyday life.

Why the weeks after treatment matter

The National Institute on Drug Abuse (NIDA, US) estimates relapse rates for substance-use disorders at 40–60%, comparable with other long-term health conditions. A return to use is not a personal failure — it can be a signal that support and structure need strengthening. Aftercare is about keeping that support in place when the structure of treatment falls away.

How support works

Support built around your recovery.

  • A dedicated sober coach for the transition out of treatment
  • Signposting to therapists and counsellors
  • Planning for triggers and high-risk situations
  • Ongoing accountability and continuity between appointments
  • Support for family members where helpful
Coaching that works alongside clinical care.

Phoenix Guardians provides coaching and recovery support. We do not provide detox, medical, psychiatric or emergency treatment, and we coordinate with the professionals who do.
